Support Coordination is a service that helps National Disability Insurance Scheme (NDIS) participants to implement and coordinate their NDIS plans. Support coordinators work with participants to:

  1. Understand their NDIS plan and goals
  2. Identify and connect with service providers
  3. Develop and implement a plan to achieve their goals
  4. Coordinate and manage services and supports
  5. Monitor progress and make adjustments as needed

VolunteSpecialist Support Coordinationering

Specialist Support Coordination is a higher level of support coordination that is designed for NDIS participants with complex or high-risk support needs. Specialist support coordinators have advanced skills and expertise in:

  • Complex case management
  • Risk management and mitigation
  • Crisis intervention and response
  • Behavioral support and intervention
  • Coordination with multiple stakeholders and services

Specialist support coordinators work closely with participants, their families, and other stakeholders to develop and implement tailored support plans that address their complex needs and ensure their safety and wellbeing.
